Institute Of Rheumatology and Clinical Immunology

  • On 6th January 1971, a humble vision took root when the Department of Medical Education granted approval for a dedicated Rheumatic Care Unit—a singular moment that would change the landscape of rheumatology in India. Within a year, that promise blossomed into daily reality as the outpatient department swung open its doors in 1972, welcoming the first patients seeking relief and expert care. By 1973, hope found even firmer footing with the establishment of a physiotherapy wing, where gentle hands and innovative exercises began restoring movement, easing pain, and renewing spirits.
  • The mid-1970s marked the dawn of leadership that would steer the unit through its formative years. In 1974, Professor A. L. Annamalai assumed the mantle of Head of Department, infusing the fledgling team with his compassionate vision. Two years later, Professor K. V. Thiruvenkadam carried that torch forward, championing clinical excellence and academic rigor. In 1978, the arrival of Professor A. N. Chandrasekaran as Reader added scholarly depth, setting the stage for the unit’s ascent as a beacon of rheumatology in India.
  • After a decade of growth and learning, 1991 heralded a watershed moment: the Rheumatology Unit launched the nation’s first DM post-doctoral program in Rheumatology, with two coveted seats that signalled India’s commitment to cultivating home-grown specialists. That spirit of pioneering achievement extended beyond the classroom in 1993, when the Journal of the Indian Rheumatology Association (JIRA) was born under Professor Chandrasekaran’s editorship—India’s inaugural platform for sharing research breakthroughs, clinical case studies, and the evolving science of autoimmune and inflammatory diseases.
  • The new millennium ushered in fresh chapters of expansion and distinction. In 2010, the DM intake doubled from two to four seats, amplifying opportunities for talented physicians to hone their expertise. Five years later, the department was elevated to a full-fledged Institute—an institutional acknowledgment of its impact on patient care, research, and education. In 2021, the dream of a standalone Rheumatology Block became reality, offering state-of-the-art facilities under one roof and reinforcing the Institute’s pledge to holistic, patient-centred treatment.
  • On 8th October 2022, half a century after that first approval on 6th January 1971, the Rheumatology Institute celebrated its Golden Jubilee—a jubilant testament to fifty years of relentless dedication, innovation, and compassion. As we honour every pioneer, every patient who walked through our corridors, and every scholar who dared to ask “why” and “how,” we stand poised to write the next fifty years of healing, discovery, and hope.

Prof. S. Ramesh MD., DM (Rheumatology) Associate Professor TNMC NO: 47308 Description: Dr. Ramesh S. serves as the Associate Professor of Rheumatology at Madras Medical College, Chennai. With 21 years of clinical experience as a consultant rheumatologist and over 25 years of dedicated teaching, he has made significant contributions to both patient care and medical education. His research focuses on lupus nephritis and vasculitis, reflecting a deep commitment to advancing rheumatologic science. A university topper in both MD and DM programs, Dr. Ramesh continues to mentor future physicians with insight, integrity, and purpose.

Assistant Professor TNMC NO: 89974 Description: Dr. Sabarinath M is a distinguished consultant in Clinical Immunology and Rheumatology, widely recognized for his expertise in managing complex autoimmune conditions such as arthritis, Spondyloarthritis, vasculitis, myositis, lupus, Sjogren’s syndrome, and scleroderma, as well as allergic and immunodeficiency disorders. He plays a pivotal role in both patient care and medical education, serving as a faculty member at Madras Medical College and RGGGH in Chennai. His academic journey includes an MBBS from Chengalpattu Government Medical College, MD from Madurai Government Medical College, and a DM in Clinical Immunology and Rheumatology from Madras Medical College. He also holds prestigious international credentials including MRCPCH (UK), MRCPCE (Edinburgh), and EULAR certifications in Pediatric Rheumatology, Musculoskeletal Ultrasound, and Imaging. Dr. Sabarinath is a prolific speaker and educator, frequently invited to National and International conferences. His research interests centre on autoimmune diseases, and he has contributed extensively to scientific literature. Fluent in English, Tamil, and Hindi, he connects with diverse patient populations. He is an active member of several professional bodies including IMA, IAP, IRA, PRSI, APLAR, and the Royal Colleges. His dedication to advancing rheumatologic care and fostering academic excellence makes him a respected figure in the medical community.

Facility & Clinical Care

OPD services are the most important part of rheumatology. OPD is in 1 floor of rheumatology block. We deal with around 500 cases per day. We receive patients from all over Tamilnadu as well as from others states. We have separate OP for new patients and patients who are on long term follow up.

Our OPD includes new patient's OPD, counter OPD, Pharmacy, physiotherapy, patient registration, patient's waiting area and X-ray unit.

Our inpatient ward is equipped with 60 beds. This includes Male ward, Female ward, day care ward and ICU Ward. We have monthly admissions of around 130 patients in female ward, 90 patients in male ward and 30 patients in ICU. Ward rounds are taken by postgraduates, assistant professors and Head of Department on daily basis for patient care. ICU is equipped with 10 beds, 4 ventilators, central oxygen supply, 3 Multi para monitors, 4 pulse oximeters and one ECG machine. Ward is equipped with procedure room for procedures like synovial fluid aspiration, intra articular injections, skin biopsy etc. We have Day care ward for the patients who are admitted for procedures as well as for biological DMARDS injections.

Physiotherapy unit is on the 6th floor of rheumatology block. Two physiotherapists are currently employed, providing their services to aid patients in rehabilitation and physical improvement. Services include electrotherapy (Interferential Therapy, TENS, Ultrasound Therapy, Infra-red radiation, Electrical stimulation, Wax-bath Therapy), Exercise therapy and play therapy.

We perform a range of immunology testing including the conventional latex agglutination test for Rheumatoid Factor (RF), C Reactive protein (CRP) and Anti-Streptolysin O (ASO);ELISA testing for auto antibodies titre Anti-Nuclear Antibody (ANA), double stranded DNA (dsDNA), Anti Cyclic Citrulinated Peptide(ACCP), Beta 2 Glyco Protein 1 (ß2 GP1), Anti Cardiolipin (ACL), Anti Neutrophil Cytoplasmic antibody (ANCA), Cryoglobulin, ANA Profile of antibodies by immunoblotting, Immunofluorescence using HEP2000 Substrate for ANA and C3 AND C4 by Nephelometry technique.Our gamut of investigations also includes Polarising microscopy, Lupus anticoagulant testing, HLA typing that is carried out using the lymphocytotoxicity (LCT) assay. All these investigations are expensive and require advanced level of technical expertise.PCR Lab is functioning in the 7th floor

Our lab strives to provide quality reporting of these speciality tests under one roof to the poor and needy rheumatological patients of RGGGH.

Musculoskeletal Ultrasound is a growing field which plays a major role in diagnosing rheumatic diseases and doing interventions in managing patients.

Nail fold capillaroscopy is a valuable diagnostic tool in rheumatology, enabling non-invasive examination of micro vascular abnormalities in patients with autoimmune diseases. It provides insights into conditions like systemic sclerosis and helps in early detection and monitoring of vascular changes. This technique plays a crucial role in optimizing patient care and treatment strategies

Our department also carries out intra articular steroids in inflammatory arthritis and intra articular hyaluronic acid in osteoarthritis.

Our facility offers a comprehensive range of biologic injections, including infliximab, adalimumab, etanercept, secukinumab, rituximab, tocilizumab, and more, to effectively manage patients with rheumatological conditions. We have well-equipped day care wards dedicated to ensuring the safe and convenient administration of these treatments.

Our golden jubilee is a landmark for us to learn what we have achieved in the past and to plan our future. Our new rheumatology block has made a great platform, on which we can improve our services for the poor patients. We have increased the inpatient bed strength to 120 and are awaiting approval. Nailfold capillaroscopy is the newest addition to our department, initiated by Prof. T. N. Tamilselvam and to be installed soon. We are now in an era of advanced molecular diagnostic testing. We have an exclusive molecular diagnostic laboratory and are focusing more on molecular research. We have a huge number of patient data which makes the ground for our research activities. We are in the process of digitalizing the records through an Electronic Medical Records system, which will give us more feasibility for planning further research.

Success story 1
  • Young girl with the diagnosis of SLE/Secondary APS with gangrene of the feet was treated with immunosuppressive therapy and surgery her limbs were salvaged
Success story 2
  • Young girl with Childhood SLE admitted with diffuse alveolar hemorrhage was saved with costly drug called IV IG and Plasma exchange along with immunosuppressive therapy
Success story 3
  • A young girl with bleeding from nose, mouth, urogenital tract, ears was diagnosed to have Ehler Danlos Syndrome and treated with medications
Success story 4
  • A bed ridden young boy with juvenile spondyloarthropathy with deformities was treated with biological therapy and physical rehabilitation started to walk independently
