History

Established in the Year: 1795

The Institute of mental health has a history of more than 230 years when the East India company appointed the surgeon Valentine Conolly as incharge of a house for accommodating persons with unsound minds.

This Institute was started in 1795 and it is one of the oldest and largest mental hospitals in Asia. After a few transitions in over six decades, the government ordered asylum in a 661/2 acre land of custodial care has developed into the present Institute of mental health, Chennai.

Memorable details of history about the Institute
  • 1 The place where the present Institute of mental health is situated now was bought by Dr. Valentine Connolly in 1794 as 46 acres of land.
  • 2 In 1799, Dr. John Underwodd, a surgeon, bought 30 acres more to establish the first mental hospital of the country
  • 3 From 1807 to 1815, Dr. Dalton expanded the inpatient facility from 20 beds to 54 beds asylum.
  • 4 In 1867, the city corporation granted permission to run a mental asylum in 661/2 acres of land in the outskirts of the city and in 1871, Dr. John Murray became the superintendent of the 145 bedded hospital
  • 5 In 1896, the city corporation took charge of the asylum and mentally ill prisoners were admitted from all the districts
  • 6 In 1922, the name of the asylum was changed to the Government Mental Hospital and Dr. Hensman and Dr. G. R. Parasuram taught the final year MBBS students for 2 weeks. Dr. Dairiam admitted patients on voluntary basis. It was during this period, the ECT and insulin therapy were started
  • 7 To combat the excess inflow of patients during 2nd world War, the hospital was expanded with various schemes like- inpatient care, male/ female wards, gardening, weaving and public health care activities through trained nurses.
  • 8 More medical equipments and medicines were obtained. Dr. T. George conducted regular review meetings and open admissions without physical restraints were made as inpatients. Criminal ward and Epilepsy ward functioned and the families were allowed to see the patients.
  • From 1948- 1957,the hospital had 14 medical officers and a host of other staff. From 1939- 1948, introduction of Electoconvulsive therapy and Insulin coma therapy, including insulin histamine therapy were given. During Phenothiazine Era, barbiturates, paraldehyde, opiates and rauwolfia serpentina were used and were discontinued after the use of chlorpromazine in 1954. In 1948, psychosurgery was done in selected cases and went out of vogue due to poor outcome and development of complications.
  • Occupation therapy (1949) and Industrial therapy center (1970) were begun which paved way for rehabilitation of patients. During this period, extension of psychiatric services to general hospitals began in the year 1949.
  • From 1957 to 1961, 2 blocks were allotted to TB patients. In 1961, a library was opened and regular Clinical Society Meetings were conducted by Dr. Sharadha Menon. Day hospital and night hospital were functioning.
  • 10 Undergraduate medical students and psychology students were given medical education. Post Graduates courses- Diploma in psychological medicine was started in 1972 and and in 1976, MD psychiatry was started at the Institute of mental health. In 2016, M.phil in clinical psychology was started. In 2021, Post basic diploma course in psychiatric nursing was started. In 1922, the name of the institution from “ Government Lunatic Asylum “ witnessed a change to “ Government Mental Hospital “. However, in 1977, it was rechristened as “ Institute of Mental Health “ by the then Honourable Chief Minister of Tamilnadu Dr. M. G. Ramachandran.
  • 11 EEG and Radiology departments started functioning. In 2017, Modified ECT administration was started. A further advancement of in non- Pharmacological management was done by starting repeated Transmagnatic Stimulation ( rTMS) in 2020. Other therapies like Biofeedback and yoga therapy were also given to the patients. In 2021, ketamine therapy was also practiced. Furthermore, since 2022, multimodal ICU started functioning in the institute.
  • In 2020, Exclusive day care center started functioning. National ID cards were issued since 2021.From 2022, Aadhar card and Unique Disability ID card ( UDID card) were also issued to the patients. By the end of 2022, the government has announced the upgrading of the institution to TIMHANS which is of international standards. In August 2023, the State Mental Health Research Cell along with the the State Mental health Video conference facilities have started functioning in the institute.
  • 12. Patients were sent to their homes on Temporary Discharge and recreational activities of all religions were coordinated through qualified social workers and inmates of the institution.
  • 13 The taboo that was attached to mental illness as shameful has changed over years to a creative and useful effort that has changed thousands of mentally challenged people. The downsizing of mental health institutions in the western countries due to unusual publicity has ended in gross violation of human rights with more wandering mentally ill and some of them becoming prisoners. This serves as a warning to our country and also encourages in admitting patients in the open Outpatient acute wards, with family members by their side atleast for a short period. This has played a major role in providing proper care in the Mental care delivery system in this modern world

My career in the Tamil Nadu Government Service, particularly my tenure as Director (FAC) and now Professor at the Institute of Mental Health (IMH), Chennai, has been dedicated to transforming public mental healthcare through compassionate, patient-centric leadership. This appraisal summarises my key contributions toward this goal. My primary focus has been on dismantling stigma and restoring dignity to patients. I led several foundational changes at IMH, including the discontinuation of patient uniforms to foster individuality and a humane recovery environment. To ensure social and civic integration, I spearheaded the successful initiative to issue Voter IDs to in-patients, enabling their participation in the 2021 Assembly Election. Furthering a holistic approach, I encouraged patients to compete in the State-level Sports Event for the differently-abled, where their remarkable efforts resulted in winning the championship trophy, powerfully showcasing their capabilities. Beyond institutional care, I have been committed to ensuring long-term rehabilitation and economic independence for recovering individuals. I conceptualised and launched vocational training programs and job fairs, creating direct employment opportunities that have successfully reintegrated many former patients into mainstream society with financial autonomy. These transformative initiatives have been recognised with several prestigious honours, including the Neeya Naana Best Personality Award (2022), the Indian Express Devi Award (2023), and The Hindu Health & Hygiene Award (2024). I view these accolades as a validation of our collective focus on innovative, human-first mental healthcare. In parallel with my administrative duties, I remain deeply engaged in academia. With over a decade of teaching experience, I actively mentor future medical professionals by conducting symposia and lectures. My contributions to the wider medical community include serving as a Quiz Master, a panelist at NIMHANS, and a speaker and chairperson at national conferences like IPSOCON and TANPSYCON. Looking forward, I will continue to advocate for and develop innovative, community-focused healthcare models that empower every individual.

Dr. P.P. Kannan, MD (Psychiatry), is Professor of Psychiatry at Madras Medical College, Chennai, with a distinguished career in psychiatric education and clinical research. He completed his MBBS in 1997 and MD (Psychiatry) in 2006, both from Madras Medical College. His academic portfolio includes organizing multiple state-level postgraduate training programs (2013–2019), conducting a 10-day research methodology workshop with ICMR support (2014), presenting papers at ANCIPS conferences (2007, 2015), and leading the TN-IPS RCBC statewide journal club for psychiatry PGs during 2021–22. He has authored and co-authored three books, including one on child mental health for teachers and parents, and published six papers in indexed journals. Dr. Kannan's teaching career spans over 15 years, including roles as Assistant Professor in Child and Adolescent Psychiatry (Institute of Child Health), Associate Professor at both the Institute of Mental Health and Kilpauk Medical College, and Professor & HOD at Chengalpattu Medical College. Since February 2021, he has served as Professor of Psychiatry at Madras Medical College/RGGGH. He is actively involved in developing training modules for mental health professionals, reflecting his commitment to academic excellence and professional development.

I'm Dr Shanthi, at present, heading the Department of Child Psychiatry as an Associate Professor at ICH &HC, Egmore, Chennai, since 2019. I've completed my MBBS from the renowned institute, MMC in the year 1996. Subsequently, I joined the Government Services in the year 2001, worked in various remote PHCs, catering to the downtrodden sector of society, and participated in several public health activities. Later on, I completed DA from MMC again, in the year 2007, and worked in Tirutani Taluk Headquarters, attending to numerous Caesarean sections and Family Planning surgeries. With an undeniable inclination towards the fascinating mental health and treating the unseen, I took up Psychiatry from yet another prestigious institution (IMH), in the year 2008 and completed in 2011. Ever since, I have worked as an Assistant Professor, sharpening my clinical skills in managing patients with mental health issues, helping with the ECTs as well when needed. I have been the state resource person for Forensic Psychiatry for nearly 2 years, training the Postgraduates throughout the state. Post promotion, as an Associate professor, I have served in various medical colleges, currently being in the famous, one of the largest in Asia, ICH &HC, Egmore, Chennai. My fields of interest are Forensic Psychiatry and Child Psychiatry, especially CSA and POCSO.

Dr Vimal completed his MBBS in Madras Medical College and finished his Postgraduation in Psychiatry from Madurai Medical College. After his senior residency, he was posted in adult psychiatry for 3 years before being posted to child psychiatry. He was working as an assistant professor in child psychiatry at the Institute of Child Health for 10 years. Currently, he is working as a Senior Consultant Psychiatrist in the Centre of Excellence for autism spectrum disorder, Chennai. His area of research includes EEG in Autism, Empathy in Autism and ADHD, Working memory in Specific Learning Disorders, and emotional disorders in adolescents. He is the recipient of National and State awards and has published multiple research papers in Autism, Learning Disorders, and ADHD.

Institute of Mental Health

  • Institute of mental health has a total bed strength of 1200; with a bed occupancy rate of 89%
  • The outpatient census is 550 with average of 20 admissions per day.
  • The Institute has an inpatient strength of around 850 on average.
  • Interventions like Electro-convulsive therapies, Repetitive Transcranial Magnetic Stimulation, and Intravenous Ketamine therapies are administered at IMH.
  • Speciality services like child guidance clinic, autism multi modal therapy unit,psychological therapies like cognitive behavioural therapy,remedial therapies.
  • Drug treatment center, opioid substitution therapy center and geriatric psychiatry care.
  • The Institute has deaddiction ward, Intensive psychiatric care unit and forensic psychiatry wing.
  • The Institute has actively involved in rehabilitation services. A number of local as well as international patients have been reunited with their family.
  • The Institute also has industrial therapy center , day care facility and half way home.

Central Facilities

  1. Prisoners Ward: Offers institutional custodial care for mentally ill prisoners.
  2. Central Library: The central library has 6895 books catering the needs of post graduates and faculties of the Institute.
  3. Central Lab: Processes about 55 blood samples per day.
  4. ECT Suite: Electroconvulsive therapy is administered to about 20 patients twice a week.
  5. Drug Treatment Center: Around 10 to 15 patients attend DTC on a daily basis.
  6. Opioid Substitution Therapy Center: About 80 patients receive substitution therapy daily.
  7. Deaddiction Ward: An average of 20 patients are admitted for deaddiction in the exclusive deaddiction ward.
  8. Intensive Psychiatric Care Unit: It is 15 bedded ward exclusively for psychiatric patients who needs close monitoring.

OP Timings: 5 units conduct OP services on weekdays from Monday to Friday, 8:00 AM to 1:00 PM.

The Institute of Mental Health at Kilpauk, Chennai, is 225 years old and one of the oldest and largest mental hospitals in Asia. Dr. Valentine Connolly of the East India Company originally acquired this place by a grant. Later, in 1794, the British Raj purchased this hospital from the East India Company.
